Just a few years ago the only choice you had for medical insurance was a Fee-For-Service plan, also known as an indemnity plan. With this plan you had to pay for your medical services, fill out a bunch of forms, then wait to be reimbursed. These plans only covered "reasonable and customary" expenses, and were very expensive.





Fee-For-Service plans are still around today but they're nowhere near as popular as managed health care plans - HMOs, PPOs, and POSs. These plans set you up with a network of health care providers and hospitals for which you pay a monthly fee and a small copayment for each doctor visit.





HMOs (Health Maintenance Organizations) are the cheapest of these plans, but you can only see doctors within your network. PPOs (Preferred Provider Organizations) and POSs (Point-of-Service Plans) cost a little more but you have the option of seeing physicians outside your network.
